我院静脉用药调配中心2018年不合理医嘱分析

摘要

目的:对我院 2018 年静脉用药调配中心(PIVAS)审核的不合理医嘱进行统计分析,促进临床合理用药。方法:对我院 PIVAS 2018 年静脉用药医嘱进行合理性审核,对发现 的不合理医嘱进行统计、分析。结果:共有 765 例不合理医嘱,占总医嘱数的 1.17%,我院不合理医嘱类型主要包括用药频次不合理、溶媒用量及选择不合理、医嘱录入错误、药物配伍以 及给药剂量不合理;通过药师干预,我院不合理医嘱率由一季度的 1.31% 下降到四季度的 1.07%。结论:通过对不合理医嘱采取措施进行干预,并不断提高药师审方能力,使得不合理医 嘱率明显降低,提高了临床合理用药水平。 Objective: To analyze irrational medical advices in the pharmacy intravenous admixture service of our hospital in 2018 and promote rational use of drugs in clinic.Methods: The medical advices of PIVAS in our hospital were checked from 2018.The statisticcal analysis was performed on irrational medical advices of PIVAS. Results: A total of 765 irrational medical advices were reviewed,accounting for 1.17% of the total medical advices. The irrational medical advices in our hospital mainly contained improper frequency , improper volumes , irrational choice of medicinal solvent , wrong instructions entry to the Internet , unreasonable drug admixtures and irrational drug dosage and so on. Through the intervention of pharmacists, the rate of irrational medical advices in our hospital dropped from 1.31% in the first quarter to 1.07% in the fourth quarter. Conclusion: By taking measures to prevent irrational medical advices and continuously improving the ability of the pharmacist, the rate of irrational medical advices was significantly reduced, and the level of clinical rational medication was improved.
