Extracorporeal membrane oxygenation for patients with cardiovascular diseases during pregnancy and postpartum

Background Despite advances in maternity care,maternal mortality due to cardiovascular diseases remains high.Extracorporeal membrane oxygenation (ECMO) has been used as a salvage mode of advanced life support,however,experience of its clinical application in obstetric patients with severe cardiovascular diseases is limited.In this study,five cases were reviewed to summarize the clinical applications in patients during pregnancy and postpartum.Methods 4 postpartum and 1 pregnant patients who received ECMO due to the cardiovascular diseases from January 2015 to July 2020 in our institution were included in this study.Statistics were analyzed retrospectively to summarize clinical characteristics,maternal and fetal outcomes,and ECMO-related complications of the patients who received ECMO.Results The median age of patients was 23 years and the median gestational weeks was 30 weeks.Duration of ECMO implantation in patients was 3-9 days.60% (3/5) of patients and 40% (2/5) of the fetus survived after ECMO.The most common complications of ECMO were hemorrhage and multiple organ dysfunction syndrome which were associated with maternal mortality.Conclusions ECMO could be used as a life support in obstetrical patients with cardiovascular diseases and it promotes maternal and fetal outcomes.
