Recent development of optical coherence tomography forpreoperative diagnosis of esophageal malignancies

摘要

Endoscopic diagnosis with histological evidence is necessaryto decide the best strategy for treating esophagealsquamous cell carcinoma and Barrett＇s-associatedneoplasia, and the recent development of endoscopictechnologies have made possible real-time information ofmalignant hallmarks. We focused on the development ofoptical coherence tomography （OCT）, the only technologythat can depict real-time cross-sectional images withhigh resolution. With the improvements in imageresolution, acquisition rate and demonstrable area ofthree-dimensional devices with Doppler capability, OCTimaging was shown to enable visualization of structural/functional alterations in the mucosal/submucosaltissue of the esophagus, resulting in more accuratepreoperative diagnosis of such malignancies. Moreover,it approved to be useful for targeting malignant areasfor biopsy and treatment as well as for predicting thetreatment effects. Therefore, further development ofthis technology is expected to overcome the currentclinical issues in management strategies of esophagealmalignancies.
