Endoscopic ultrasound-guided biliary drainage as analternative to percutaneous drainage and surgical bypass

摘要

Endoscopic retrograde cholangiopancreatography hadbeen a treatment modality of choice for both benignand malignant biliary tract obstruction for more thanhalf century, with a very high clinical success rate andlow complications. But in certain circumstances, suchas advanced and locally advanced pancreatobiliarymalignancies （pancreatic cancer, cholangiocarcinoma,ampullary tumor） and tight benign strictures,endoscopic retrograde cholangiopancreatography（ERCP） fails. Up to this point, the only alternativeinterventions for these conditions were percutaneoustranshepatic biliary drainage or surgery. Endoscopicultrasound guided interventions was introducedfor a couple decades with the better visualization andachievement of the pancreatobiliary tract. And it＇s still inthe process of ongoing development. The inventionsof new techniques and accessories lead to morefeasibility of high-ended procedures. Endoscopicultrasound guided biliary drainage was a noveltreatment modality for the patient who failed ERCPwith the less invasive technique comparing to surgicalbypass. The technical and clinical success was highwith acceptable complications. Regarded the ability todrain the biliary tract internally without an exploratorylaparotomy, this treatment modality became a veryinteresting procedures for many endosonographers,worldwide, in a short period. We have reviewed theliterature and suggest that endoscopic ultrasoundguidedbiliary drainage is also an option, and one witha high probability of success, for biliary drainage in thepatients who failed conventional endoscopic drainage.
